Check List Prior To Chemotherapy

1. Recent blood investigation reports (if not done recently ,kindly ask your oncologist).
2. Previous discharge summary.
3. Details of last chemotherapy.
4. Allergy cards/details of drugs you are allergic to.
5. Details of other medications being taken for associated diseases e.g. antihypertensive, diabetes, thyroid medicines etc.
6. Details of any alternative treatment, if being taken.
7. Check for your appointment slot for chemotherapy.
8. In case your treatment is covered under any insurance, kindly check if prior approvals are ready.
